  • Once we have your information from the screening form, we’ll reach out to you about next steps and scheduling.

  • Once you are scheduled, you are ready to meet with your provider and start your care journey.

Insurance & Payment

Please note: It is the patient’s responsibility to verify insurance eligibility and benefits prior to services.

We are currently in the process of credentialing with most major insurance providers, including:
BCBS, Cigna/Evernorth, UHC/Optum, Humana, Wellpoint, Ambetter, and Blucare.
Applications can take up to 120 days to process.

Until credentialing is complete, we are accepting self-pay patients.
